1. Factory Reset the unit, kept the default ip address (192.168.15.1)
  2. Set the host name to SPA122_3006
  3. Set a new Admin password
  4. Enabled Web Accesses Management
  5. Provisioned ext.3006
  6. Confirmed it worked, had dial tone and could make an outgoing call.
  7. I upgraded the firmware to 1.3.3 (12/2013) from 1.3.1 (12/2012).
  8. Confirmed it worked, had dial tone and could make an outgoing call.
  9. Enabled Bridge Mode
  10. Confirmed it worked, had dial tone and could make an outgoing call.
  11. Verified that it would work on the State Network and our fiber connection
  12. Verified IP pass through from the WAN to the LAN port, pulled a DHCP address
